Soon you will be coming to St. Joseph's Hospital, a fully accredited teaching hospital affiliated with the University of Western Ontario. Our hospital is part of St. Joseph's Health Care, London.

To simplify your registration, please fill in the accompanying form and return it to us as soon as possible. In order to view/print the form you will require Adobe Reader.
